Life Camp Summer Camp Registration begins April 30, 2010. At the time of registration, you must have the following:

___ Registration Fee(s)

___ Weekly Payment (Registration is not complete without weekly payment)

___ Life Camp Health Record

___ Camper Health History

___ Shot / Immunization Records (needed for ALL non-Maryland students)

___ Emergency Contact and Authorized Pick Up Form

___ Life Camp Release Form

___ Life Camp Acknowledgement Form
